The information on this website is educational. It cannot diagnose an eye condition, determine whether a procedure is right for you, or replace an examination. Your clinician will discuss benefits, risks, alternatives, and a plan for your individual eyes.
For appointments or personal questions, call (813) 330-0574. For medical records, registration documents, or other health information, ask the team for its approved process.
This website is not monitored for urgent concerns. Sudden vision loss, new flashes or a sudden increase in floaters, a curtain or shadow in your vision, or severe eye pain needs immediate medical assessment. Contact an eye-care professional or go to an emergency department now. Call 911 for a medical emergency.
How these
pages work.
The appointment page includes a Google Forms request form and a direct link to open it. Information you submit is handled through the practice’s Google Workspace form and sent to its appointment inbox. A request is not a confirmed appointment. Keep your request brief and do not include medical records, birth dates, insurance numbers, payment information, or urgent concerns. The educational self-checks are reading guides: they have no answers to submit, no score, and no saved responses.
The website’s own scripts control navigation and care filters. They do not read or store form answers. The financing page offers an optional Cherry calculator, loaded only when you choose to load its tools. Cherry and its service providers may use analytics and process technical information under their own policies. The website does not read financing applications. Google Forms, hosting, and access services may also process information separately under their own privacy practices.
Telephone links open your device’s calling app. Directions and educational references open external services, which have their own privacy practices. For information about how the practice handles medical records, please contact the office.
